Expand ASN1_ITEM_ref and ASN1_ITEM_ptr macros - no change in generated

assembly. Of particular interest is ASN1_ITEM_ptr which does nothing
and resulted in code like:

  if (method->it)
    ASN1_ITEM_free(..., ASN1_ITEM_ptr(method->it));
