This commit copies a small hidden bugfix from the OpenSSL commit
https://github.com/openssl/openssl/commit/fbb7b33b
the rest of which is merely cosmetics.
I discovered the bug independently while documenting sk_find(3).
Keep the library's idea of when an empty stack or a one-element stack
is sorted and when it is not bug-compatible with OpenSSL, even though
in fact, empty and one-element stacks are of course always sorted.

There are a few instances where #if 1 is removed but the code remains.
Based on the following OpenSSL commits. Some of the commits weren't
strictly deletions so they are going to be split up into separate commits.

Remove the openssl public includes from cryptlib.h and add a small number
of includes into the source files that actually need them. While here,
also sort/group/tidy the includes.

ASN1_unpack_string() from the NO_ASN1_OLD purge; turns out that some Ruby
extensions still use them, unfortunately, so we need to give them some more
time to catch up on ``recent'' interfaces.
Riding on the libcrypto major bump.

As an excuse gift, the memory leaks on failure in resurrected a_set.c have

We don't expect any reasonable software to stick to these interfaces, so better
clean up the view and unifdef -DNO_ASN1_OLD.
The astute reader will notice the existence of NO_OLD_ASN1 which serves a
similar purpose, but is more entangled. Its time will come, soon.
