The 'reset' applet was changed to be NOFORK for WIN32. Ensure
that it remains NOEXEC in the POSIX build.
The default POSIX configuration doesn't enable any of the features
which need this information (PREFER_APPLETS, SH_STANDALONE and
SH_NOFORK).

Fix merge conflicts in coreutils/ls.c and shell/ash.c.

The 'reset' applet attempts to reset the screen. It's more thorough
than 'clear'.
Since it tries to reset the screen buffer for the current process
it has to be a NOFORK applet.
To run the Windows executable C:/Windows/System32/reset.exe use the
full path, as the 'reset' applet will take precedence.

The applets with "<applet> [opts] PROG ARGS" API very quickly exec
another program, noexec is okay for them:
chpst/envdir/envuidgid/softlimit/setuidgid
chroot
chrt
ionice
nice
nohup
setarch/linux32/linux64
taskset
cttyhack
"reset" and "sulogin" applets don't have this form, but also exec
another program at once, thus made noexec too.

Those two spaces after tab have no effect, and always a nuisance when editing.

This change retains "or later" state! No licensing _changes_ here,
only form is adjusted (article, space between "GPL" and "v2" and so on).

- Rewrite kbd_mode and setconsole
- Introduce and use console_make_active() and xopen_xwrite_close()
- honour buffer-reservation method as set by the user (dumpkmap, loadkmap)

busybox.h which slowed compiles. I left only what was needed and then fixed up
all the apps to include their own header files. I also fixed naming for pwd.h
and grp.h functions. Tested to compile and run with libc5, glibc, and uClibc.
